Last July, a community hospital called me at 4 p.m. on a Friday. Here’s the thing: the “refurbished = unreliable” reputation comes from a time when “refurbished” could mean wiped down and resold. That era is mostly gone—at least, that’s been my experience with ISO-certified refurbishers and Hologic’s own certified pre-owned program. A properly refurbished Hologic mammography machine will have replaced parts, calibration records, and a test image set. A gray-market unit may have none of those.
But reliable hardware is only half of the clinical risk equation. A mammography system is a screening tool. When it malfunctions, patients come back for repeat images, which means extra radiation, extra anxiety, and an extra appointment slot you didn’t plan for. MQSA and ACR accreditation requirements don’t stop applying just because a machine was “previously used.” A refurbished unit has to meet the same quality standards as a new one. So ask for evidence. If the refurbisher can’t provide recent phantom image test results or state inspection records, move on.
Support: The Difference That Makes the Decision
I have mixed feelings about refurbished equipment. On one hand, it expands access for clinics that would otherwise be priced out. On the other hand, support on refurbished units varies wildly. Some third-party sellers offer a solid two-year warranty and guaranteed parts; others offer a one-year promise and then go quiet.
The question isn’t “What happens if the machine breaks?” It’s “What does the response look like when it does?” From our internal data across 60+ equipment purchases, the fastest third-party refurbisher service call we ever got was four hours. The slowest was three days. For a diagnostic imaging department, three days of downtime is a serious event—especially if the machine is the only one in the building.
New Hologic equipment comes with direct manufacturer support, scheduled maintenance, and a predictable parts pipeline. That’s a different risk profile. If your center has backup coverage from a neighboring hospital, refurbished might be fine. If you’re a standalone clinic, support matters more than price.
What Hologic Actually Makes (and Doesn’t)

At the end of the day, I don’t look at this as a money-versus-quality battle. I look at it as a prevention-versus-correction problem. A new system is a prevention strategy: fewer callbacks, faster workflows, better software, and predictable support. A refurbished system is a correction strategy: lower initial cost, but more variables to manage.
There’s a place for both. If you’re a mobile screening van, if you’re a rural clinic with a low patient volume, or if you need a backup unit to cover your primary, a Hologic-certified refurbished machine can be a smart move. But if you’re running a high-volume breast center and your name depends on getting patients in and out with clear, reliable images, the added cost of new equipment starts to look like insurance—not an expense.
